The below information will be used for the completion of member’s cards and other Yacht Club functions.  Address and telephone numbers DO NOT have to be placed on this form.  Yacht Club information will be disseminated by the most effective and cost saving way possible or by the member(s) request as stated below.  Members must adhere to the current Summit North Yacht Club By-laws and Amendments.  You must sign the membership agreement to validate your application.

I further waive all claims for damages or restitution in any form for injuries or damages to persons or property sustained from any cause whatsoever in connection with any function or activities sponsored by the Summit North Yacht Club.  I specifically relinquish all rights to suit or legal action of any sort against the Summit North Yacht Club, its officers or members when acting in the club capacity, in relation to any damages or injuries.
